I agree to allow Chicago Music & Acting Academy to take photos, videos, and/or recordings of my child during lessons, recitals, and other events for archival purposes/promotional use that may also be displayed on our website.

I agree to pay amount due by first day of class or arrange a payment plan.

After submitting your registration, you'll be asked to remit a $50 deposit via PayPal, a debit or credit card (or Venmo if registering on a mobile device). If paying by check: Returned checks will incur a $30 fee. Checks will not be accepted if more than two checks are returned.

Any discounts will be applied at time of billing.

Chicago Music & Acting Academy is not responsible for any bodily injuries or for any personal belongings left behind. No refunds can be issued at this time.
